Home Health Services

In the comfort of your own home, regaining your independence is the heart of home health services. Under the direction of your physician, a RN Case Manager will determine a plan of care.

Services included but are not limited to:

• Skilled Nurse (LPN, RN)
• Physical, Speech and Occupational Therapy
• Personal Care (Home Health Aide)
• Wound Care
• Diabetic Care
• Lab Work
• Medication Monitoring
• Blood Pressure Monitoring
• Licensed Social Worker

Payment

Indian Territory will be more than happy to file Medicare, Medicaid, and Private Insurance claims for service. Private Pay options are also available.
